The role of the Health Outreach and Inclusion Service Link Worker is a
new MQI initiative developed in partnership with the Inclusion Health
team from St James Hospital. Many socially excluded people have low
health aspirations and poor expectations of services and get little
opportunity to shape their care.
The focus of this role is to create strong relationships based on
trust and to maintain communication with individuals experiencing
homelessness; to engage and facilitate their access from A&E and
Primary care to specialist treatment; and offer support and follow-up
appointments as appropriate.
We believe that improving engagement and communication with both,
clients and healthcare providers will promote an integrated care
approach, improve health outcomes and minimize costs.
